Neuro E-Consult® for Care Transitions

Neuro E-Consult® is an innovative form of telemedicine that takes remote and digital care to the next level. Our form of telehealth places neurological care “everywhere” from one-time neuro consultations within acute care settings to to the outpatient clinic and beyond.

The focus is for the health care team efficiently transact neurological care on behalf of a patients being “transitioned” between different care settings and or from one care environment to their home base.

Telemedicine is the use of telecommunication systems to deliver healthcare at a distance” as per the Cochrane Database Syst Rev. 2015: 2015 (9): CD00298 in an article by Flodgren et al.

Neuuro E-Consult® utilizes a set of algorithms to synchronize and streamline the consultation process with a specialized communication flow. Pertinent neuro communication and knowledge is shared across neurology care environment, between physicians, patients and important stakeholders in the neurological care process.

Our modules fit into existing health care processes and they help to ensure adequate communications across care setting and between providers on the care team.

Neuro E-Consult®  also facilitates early supported discharges to help improve transitions between hospital and the community, thus decreasing time in hospital time and providing enhanced neurological care continuity in the home setting or other facility.

Health care encounters that are facilitated by Neuro E-Consult® can occur in person, via audio-visual dissemination or online.  Critical nervous-system related information is standardized to be included in all communications and encounters along each point in an individual’s neurological care pathway.

Our neuro patient care education is seamlessly incorporated into the normal communications applicable and accessible to the patient. Further, customized education material is suited for the specific neurological clinical circumstances of the individual patient.
